Chest reconstruction

Recovery of the mammary gland after partial or complete removal (mastectomy) allows a return to full life!

Aesthetic breast reconstruction surgery is one of the important areas that enable women who have undergone breast cancer surgery to reshape their breasts and regain their confidence. Nowadays, breast cancer is quite common among women and in most cases, systemic treatment involves mastectomy – mammary gland removal.

When is breast repair recommended?

In some cases, the reconstruction is done immediately after the mastectomy, so that the patient wakes up with the restored breasts after the anesthesia. However, simultaneous operations are not always possible and everything depends on the complexity of the case, in particular, when after the removal of the chest glands radiation therapy sessions are provided. Then, the operation to restore the breast is planned after the treatment. The breast implant can be placed six month later or even a year after the mastectomy, which helps to achieve a more beautiful result.

During this time, the tissue will heal as much as possible, the lymph flow will improve and a large implant can be placed.

Before mastectomy, the patient should consult a doctor and learn about breast repair possibilities that can be applied to her case. The doctor will tell you about all the risks, pros and cons of each method, the types of implants and their features.

Recovery of the breast after a mastectomy has several advantages:

  • Reduces the risk of relapse;
  • The imbalance is eliminated and the load on the spine is balanced out;
  • Restores the appearance of the chest;
  • Reconstruction of the breast makes it possible to look better and not to wear a special prothesis;
  • The emotional state is improving;
  • Patient can return to a full life;
  • The sense of integrity returns.

In addition, the restoration of the bust allows you to get rid of psychological discomfort and cope with the depression caused by removal. After mastectomy, many women have complexes that negatively affect their self – esteem and acceptance of themselves. The psychological state of the patient after breast removal has a negative impact on her life, work and family. There is a violation of body proportions that were previously associated with femininity, which leads to deep feelings, anxieties, fears.

Important! When a woman knows that her breast will be restored, her further treatment is easier and she feels more comfortable.

Proved that breast recovery: does not increase the risk of relapse and does not change the forecast!

Under the guidance of plastic surgeon Alexander Bebykh, the most complicated reconstructive interventions are performed and breast reconstruction can be performed using the following methods:

Patchwork reconstruction of the chest.

In this case, the patient’s tissue is used. The method is quite traumatic and has a long rehabilitation period. The doctor takes complex tissue complexes from other parts of the body (abdomen, back, buttocks, inner thighs) and moves them to the place of the removed bust on the nourishing muscle leg or in a free version, when the nourishing flap vessels are sewn to the vessels in the chest area using a microscope (microsurgical operation).

Important! The method is used when the breast is removed together with the pectoral muscle, or when there is extensive thickening of the skin after radiation therapy.

Breast reconstruction with espander and implants.

Espander is a temporary device that is placed on the chest under the chest muscle. The device is used to expand the created soft tissue pocket to place the implant there in the future. Espanders come in different shapes and sizes. Usually, espanders have special holes for filling (port). The device is filled within one minute and the fluid that is placed is limited by the degree of stretch of the patient’s skin. The standard volume for each step of filling the expander is 50 ml of physiological solution (equivalent to 10 teaspoons). Upon completion of the device filling, the patient is prepared for the second stage of reconstruction – replacement of the expander with an implant.

Combination operations (thoracic dorsal flap and implant placement).

In this case, the patient’s tissues (from the lower abdomen) and the implant are used to adjust the required volume. Implants are placed under the muscular and skin flap. Such surgery allows for recovery in one stage. With this technique, the flap includes skin, subcutaneous fat and blood vessels, namely, the deep lower epigastric artery and veins. There are muscle in the flap.

Restoration of breast shape and volume with the help of lipofilling (filling with fat cells) – breast shape is created at the expense of the patient’s own fat, which is transplanted from donor zones (thighs, back, abdomen).

The first stage – Restoration of the shape and volume of the chest. The restored bust is of moderate size and is usually “high”, which will lead to healthy breast surgery to create symmetry. Then, the surgeon increases or decreases the healthy breast and thus corrects the ptosis. Both breasts are operated on simultaneously.

The second stage – evaluation of a possible result. Different tissue reactions may be observed after the operation. Usually this happens after radiation therapy. The result will be visible 2 months after recovery.

The final stage is the reconstruction of the nipple and areola. If asymmetry of the breasts is observed, this procedure is recommended. The reconstruction can be performed by a surgeon using a part of the nipple of a healthy breast, a prosthesis or a micropigmentation (skin staining).

Important! The restored nipple will have no sensitivity and will have no nerve endings.

After the operation, the patient stays in the clinic under supervision for up to 7 days (depending on the type of operation). After discharge, the woman should follow all recommendations for the care of the breast and drain tubes in order to prevent infection and complications and come for regular check-ups. The drain is installed during the operation and removed, depending on how quickly the patient recovers.

To reduce pain, you will need to take painkillers first. To reduce swelling, the patient must wear compression underwear. Scars will tighten after 6-8 weeks and will become almost invisible over time. After 2 months, the woman can go back to her old life. During the recovery period, no heavy lifting, physical activity, sauna or solarium is allowed.
